In environments where failures can lead to catastrophic consequences—chemical processing, power generation, critical infrastructure—AI systems demand more than standard IT risk approaches.
I have worked with organizations exploring these risks and others, and a concerning pattern emerges: we're integrating powerful, opaque AI capabilities into safety-critical environments without commensurate safety controls.
Below are five fundamental risk dimensions that every leader in high-consequence industries must address before deploying AI in operations.
1. System Risk
Many organizations appear to be treating AI as just another IT application rather than an engineered system requiring rigorous safety controls. Unlike conventional software with deterministic behaviours, AI systems exhibit emergent properties that demand the same engineering rigour we apply to other safety-critical systems. When deployed in environments where failures could have catastrophic consequences, this mis-characterization creates dangerous blind spots.
2. Context Risk
How do we know an AI system has sufficient context to provide accurate, safe recommendations? In high-risk environments, contextual gaps can be deadly. AI assistants might provide technically sound advice while missing crucial site-specific factors like recent equipment modifications, temporary operating constraints, or concurrent activities that introduce additional risks.
3. Agentic Risk
As AI evolves from passive advisor to active agent—executing commands across multiple systems—human oversight becomes increasingly challenging. When AI agents perform complex sequences of operations with limited transparency, how can operators maintain effective supervision? This opacity creates new risks that our current safeguards weren't designed to address.
4. Predictive Risk
AI excels at finding patterns, but correlation isn't causation. Without scientific rigour to validate whether identified patterns represent genuine causal mechanisms or mere statistical artifacts, organizations risk making critical operational decisions based on spurious correlations. In safety-critical environments, this lack of causal understanding undermines the foundation of risk management.
5. Model Risk
AI systems are inherently stochastic—producing outputs based on probabilistic processes rather than deterministic calculations. Yet high-consequence engineering decisions typically demand certainty within established safety margins. This fundamental tension between AI's probabilistic nature and engineering's deterministic requirements creates technical reliability challenges that must be addressed before deployment in safety-critical applications.

Successfully integrating AI into highly-regulated industries requires interdisciplinary collaboration between AI specialists, domain experts, safety engineers, and regulators. We need frameworks that treat AI not merely as another IT implementation, but as a fundamentally new class of engineered system requiring commensurate rigour.
Organizations that proactively address these challenges will be positioned to realize AI's benefits while avoiding potentially catastrophic failures in environments where reliability and safety remain paramount.
